SUBMENTAL LIPOSUCTION

What is submental liposuction?

Submental liposuction is a surgical procedure performed to remove excess fat tissue from beneath the chin (submental area) and, when needed, around the jawline using thin cannulas. The goal is to create a more defined jawline, a more balanced neck profile, and a more refined lower-face appearance.

This procedure is especially effective when fullness in the “double chin” area is mainly caused by fat. However, not every double chin is due to fat alone; skin laxity, muscle bands, a recessed chin, or other anatomical features may also contribute.

 

Who is a good candidate?

You may be a suitable candidate if you have one or more of the following:

  • Noticeable fat accumulation / fullness under the chin

  • Persistent fullness under the chin despite weight loss

  • Poor definition of the chin-neck angle on profile view

  • Genetic predisposition to submental fullness at a younger age

  • Desire for contour refinement with good to moderate skin elasticity

Suitability is determined based on skin elasticity, the amount of fat tissue, platysma muscle condition, chin projection, and overall face-neck anatomy.

 

Is submental liposuction enough for every patient?

No. Fullness under the chin is not always caused only by excess fat. In some patients, the appearance may also be influenced by:

  • Skin laxity

  • Neck muscle bands (platysma)

  • Prominent salivary glands

  • A retruded chin (microgenia/retrognathia)

For this reason, submental liposuction alone may be sufficient in some patients, while others may benefit more from neck lift, chin procedures, skin tightening, or combined approaches.

 

Which areas can be treated?

Depending on the treatment plan, the following areas may be addressed:

  • Under the chin (submental region)

  • Limited areas along the jawline (jawline contouring)

  • Upper neck fat accumulation (in suitable patients)

The goal is not only fat removal, but also creating a natural and balanced contour between the face and neck.

 

Will there be scars (incisions)?

Submental liposuction usually involves very small incisions for cannula entry. These are often placed:

  • Within the natural crease under the chin

  • Under/behind the earlobes or other less noticeable areas (depending on technique)

These marks usually become less noticeable over time, but the procedure is not completely scar-free.

 

Anesthesia and surgery time

Submental liposuction may be performed under local anesthesia (with sedation if needed) or general anesthesia, depending on the patient and the extent of the procedure. It typically takes about 30–90 minutes (varies by patient and any additional procedures).

Combined procedures (e.g., facelift/neck lift/chin procedures) may increase the duration.

 

Pre-operative preparation

Common recommendations include:

  • Adjusting blood thinners and certain supplements as advised

  • If you smoke: reducing/stopping smoking to support wound healing and skin circulation

  • Photography and evaluation of the profile / chin-neck angle

  • Required tests and anesthesia evaluation (depending on procedure extent)

  • Planning for a post-operative chin-neck compression band/garment

 

Recovery and return to daily activities

A general timeline (varies by patient):

  • First 2–5 days: Swelling, tenderness, bruising, and tightness may occur.

  • First week: A compression band/garment may be recommended (duration varies by patient).

  • 5–10 days: Many patients can return to social activities (depending on bruising/swelling visibility).

  • 2–4 weeks: Swelling decreases significantly and contour becomes more apparent.

  • 1–3 months: The result can be assessed more accurately (especially as skin adapts and swelling resolves).

Compression use, avoiding movements that strain the head/neck area, and follow-up visits help support healing.

 

Safety, risks, and important considerations

Submental liposuction is generally safe in properly selected patients, but like any surgery, it carries risks. Commonly discussed risks include:

  • Swelling, bruising, tenderness

  • Temporary asymmetry

  • Contour irregularity / surface waviness

  • Aesthetic dissatisfaction due to undercorrection or overcorrection

  • Fluid collection (seroma), bleeding

  • Infection

  • Changes in skin sensation / numbness (often temporary)

  • Insufficient skin tightening in patients with poor skin elasticity

  • Rarely, the need for revision surgery

 

Important notes:

  • The quality of the result depends not only on how much fat is removed, but also on skin elasticity and underlying anatomy.

  • Overly aggressive fat removal increases the risk of irregularities and an unnatural appearance; conservative and balanced contouring is important.

  • Smoking may negatively affect healing and skin quality.

 

Rare warning signs requiring urgent evaluation:
Rapidly increasing swelling, marked one-sided firmness/bleeding, high fever, severe pain, or unexpected breathing/swallowing difficulties require prompt medical evaluation.

Frequently Asked Questions (FAQ)

1) Does submental liposuction cause weight loss?

No. This is not a weight-loss surgery. The goal is to improve under-chin contour and profile appearance.

 

2) Are the results permanent?

The fat cells removed do not come back, so results can be long-lasting. However, weight gain, aging, and skin laxity may affect the appearance over time.

 

3) Can I still have it if I have loose skin?

Yes, but if skin laxity is significant, liposuction alone may not provide an ideal result. In such cases, skin tightening or a neck lift may be considered.

 

4) Does submental liposuction define the jawline?

Yes, in suitable patients. Especially when excess fat is the main issue, the chin-neck angle and jawline appearance can become more defined.

 

5) Is the procedure painful?

Severe pain is not usually expected. Most patients experience tenderness, tightness, and soreness, which are generally manageable with medication.

 

6) When can I return to work?

For desk-based jobs, many patients return within 3–7 days. The exact timing varies depending on bruising/swelling visibility and compression use.

 

7) How long do I need to wear the compression band?

This depends on the surgical technique and findings. Many patients are advised to use it more intensively in the first days, then gradually reduce use. Your exact plan is determined during follow-up visits.

 

8) Will I see the result immediately?

The early appearance can be misleading due to swelling. Contour usually becomes clearer over weeks, and a more accurate assessment may take 1–3 months.

 

9) Can asymmetry occur?

Temporary asymmetry may occur early due to swelling. Permanent asymmetry is uncommon, but natural asymmetry is always possible in the face-neck area.

 

10) Can it replace a neck lift?

No, not always. Submental liposuction targets excess fat; if there is significant skin laxity or platysma banding, a neck lift may be more appropriate or may need to be combined.

 

11) Will the scars be noticeable?

Cannula entry marks are usually very small and placed in less visible areas. In most patients, they become less noticeable over time.

 

12) Will I need revision surgery?

Most patients do not. However, revision may occasionally be needed for contour irregularity, undercorrection, or asymmetry.

 

13) Can it be combined with a chin procedure?

Yes. In some patients, chin augmentation/reshaping may be combined to improve profile balance. This is decided during consultation.

 

14) Is it suitable for younger patients?

Yes. It can be an excellent option in younger patients with good skin elasticity and genetic submental fullness.
